Visit the Delta Dental enrollment page: deltadentalcoversme.com

Fill out basic personal information and click “Get My Quote.”

Select a plan and click “Enroll.”

Click “Purchase Here.”

Create your account.

In the Broker Name drop-down box, select David Glasker or Jacob Glasker.

Fill out the applicant information.

Fill out billing and submit for payment (Visa, Mastercard, and Discover accepted).

Your account is created and your ID card should be available.
